Outpatient and Extended Care Programs

The program provides individualized treatment and supportive service planning directed at improving an individual's ability to function more effectively in the community. Individualized service plans are comprehensive and target changes in a variety of interrelated areas. These areas may include facilitating adaptation of a chemical-free lifestyle, improvement in overall functioning, increasing self-sufficiency and be a healthy, contributing member of the community.
